Users who have submitted correctly are marked with "O".
Users who have not submitted are marked with "X".
Users who have submitted with errors or omissions are marked with "△", along with the specific incorrectly filled items of the W-8BEN.

6b: If item 6a is not filled out on the W-8BEN form, a check is required.
Signature: An electronic or non-electronic signature is required on the right side of "Sign Here" at the bottom of the W-8BEN form.
Signature Check Box: The checkbox on the right side of "Sign Here" at the bottom of the W-8BEN form needs to be checked.
Signature Date: The signature date should be entered in the format of two digits for the month, two digits for the day, and four digits for the year in the "Date" section at the bottom of the W-8BEN form.
Date of Birth: The date of birth should be entered in the format of two digits for the month, two digits for the day, and four digits for the year in the middle section of the W-8BEN form under "Date of Birth."
Print name of signer: The name of the recipient should be provided in the "Print name of signer" section at the bottom of the W-8BEN form.
Users who have not submitted or have made errors/omissions will not be able to receive prizes, so please check and submit according to the submission period.
